Resumo Objetivou-se avaliar as propriedades psicométricas de um instrumento para avaliar a integralidade do cuidado por dentistas, usando uma combinação da teoria clássica e teoria da resposta ao item. Um instrumento com 46 itens foi desenvolvido e testado por um painel de experts, seguido por um teste piloto e aplicação em 187 dentistas da atenção primária de uma grande cidade brasileira. Os seguintes critérios foram utilizados para avaliar os 46 itens: aceitabilidade, consistência interna, estabilidade temporal, correlação inter-item e correlação tetracórica. Essa avaliação resultou em uma versão reduzida de 11 itens que preencheram todos os critérios descritos acima. A estabilidade temporal foi medida utilizando o Cohen Kappa. Os 11 itens apresentaram valores maiores que 0,5. O alfa de Cronbach foi de 0,72. Nenhum dos 11 itens apresentou perda na distribuição das respostas, e o modelo que considera a discriminação como variante se adequa melhor aos dados do que o modelo que considera a discriminação como um parâmetro constante (p <0,001). Curvas características mostraram que 54,5% dos itens pode ser considerado difícil, ou seja, apenas os dentistas com uma boa compreensão da integralidade respondeu favoravelmente. O instrumento de 11 itens para avaliar a integralidade do cuidado por dentistas é considerado como tendo boas propriedades psicométricas.

Abstract This study aimed to evaluate the psychometric properties of an instrument to assess comprehensiveness of care from dentists using a combination of classical test theory and item response theory. A 46-item instrument was developed and tested by a panel of experts, followed by a pilot test and administration to 187 primary care dentists in a large Brazilian city. The 46 items were evaluated using the following criteria: acceptability, internal consistency, temporal stability, inter-item correlation, and tetrachoric correlation. This evaluation led to a shortened version consisting of 11 items that met all the criteria previously described. The temporal stability was measured using Cohen’s kappa, and all 11 items presented values greater than 0.5. The Cronbach’s alpha value was 0.72. None of the 11 items had missing data on the distribution of responses, and the model considering the discrimination as varying fit the data better than the model considering discrimination as a constant parameter (p<0.001). Item characteristic curves showed that 54.5% of items could be considered difficult, i.e., only dentists with a good understanding of comprehensiveness responded favorably. The 11-item instrument to assess comprehensiveness of care by dentists is considered to have good psychometric properties.

Comprehensiveness relates both to the scope of services offered and to a holistic clinical approach. The aim of this study was to identify the perception of Brazilian dentists regarding key concepts of comprehensiveness and its importance in primary health care oral health services performed in the public sector. Focus groups were conducted to explore three main concepts, ‘patient welcoming’, ‘bonding’ and ‘quality of care’, as well as their aspects. In total, there were four focus groups comprising seven general dentists, all of whom had at least two years of experience in primary care, from municipalities of varying sizes. Discussions of approximately 60 minutes were conducted, audio-recorded and transcribed. The data were analysed by qualitative thematic analysis, in line with the framework approach. The general dentists reported that they are aware that in the Brazilian Health System, the onset of patient care by primary health care services must happen through ‘patient welcoming.’ Nevertheless, they suggested that this action is often performed poorly due to the large volume of patients. Although they knew the importance of ‘bonding’, they realised that there is a lack of education preparing professionals to address interpersonal and social issues. They were aware that ‘quality of care’ is related to multiple factors. Comprehensiveness, as an approach in dental public health practices, needs to be enhanced, and there is evidence that primary care dentists are aware of the need for such an approach.

OBJETIVO: Descrever o perfil dos cirurgiões-dentistas da equipe de saúde bucal de uma microrregião de saúde de Minas Gerais, Brasil. MÉTODOS: Estudo transversal com 48 cirurgiões-dentistas de 16 municípios, cuja coleta de dados foi realizada por questionário. As variáveis consideradas foram: tempo de formação em anos e salário mensal em reais (centroides), cumprimento da jornada de trabalho semanal, modalidade de incorporação na equipe de saúde bucal, curso de pós-graduação, participação social e participação em curso na Saúde da família. Todas as variáveis foram utilizadas para agrupar os cirurgiões-dentistas por meio da análise de segmentação TwoStep Cluster com distância euclidiana quadrática, com p<0,05 de significância. RESULTADOS: Formaram-se três clusters que se diferenciam principalmente em relação ao tempo de formação profissional (p=0,007), jornada cumprida de trabalho (p<0,001), pós-graduação (p<0,001) e participação social (p=0,031). O primeiro reuniu cirurgiões-dentistas com menor tempo de formação, salário intermediário que declararam cumprir a jornada de trabalho e que não possuem curso de pós-graduação; o segundo reuniu cirurgiões-dentistas com os maiores salários e tempo de formado que possuem curso de pós-graduação e o último foi formado por cirurgiões-dentistas com tempo de formação intermediário, os menores salários, que cumprem a jornada de trabalho, já realizaram curso de capacitação/treinamento na Estratégia Saúde da Família e não tem participação social. CONCLUSÃO: Os cirurgiões-dentistas da Estratégia Saúde da Família são diferentes em relação à jornada de trabalho semanal, curso de pós-graduação e participação social no município, sendo que o tempo de formação pode ser uma característica crucial para apontar essas diferenças.

OBJECTIVE: To describe the profile of dentists in oral health team of the micro-region of Minas Gerais, Brazil. METHODS: Cross-sectional study with 48 dentists in 16 municipalities, in which data collection was conducted by questionnaire. The variables considered for the study were: training time in years, and monthly real salary (centroid), in compliance with weekly working hours, mode of incorporation into the oral health team, postgraduate course, social participation and ongoing involvement in Family Health. All variables were used to group dentists by Two Step Cluster analysis segmentation, with p <0.05 significance level. RESULTS: They formed three clusters that differed mainly in relation to training time in years (p = 0.007), hours worked (p <0.001), postgraduate course (p <0.001) and social participation (p = 0.031). The first cluster grouped dentists with less training time, receiving an intermediate salary to comply with the stated working hours and lack of post-graduate course, the second grouped dentists with the highest salaries and years of graduation who have post-graduate course and the last was composed of dentists with intermediary training time, the lowest salaries that complied with the working hours, have attended the training course / training in the Family Health Strategy and have no social participation. CONCLUSION: Dentists in the Family Health Strategy differ as regards weekly working hours, postgraduate course and social participation in the city, and the training time can be a critical feature to point out these differences.

O objetivo deste artigo é discutir a inclusão da saúde bucal na Estratégia Saúde da Família, em cidades de uma microrregião do sudeste do Brasil, sob a perspectiva dos recursos humanos. Foram aplicados questionários aos integrantes do serviço de saúde bucal: dentistas, auxiliares e gestores. O instrumento considerou três dimensões: como e porque se adotou o novo modelo, como o serviço está se reorganizando e que avanços foram percebidos pelos profissionais. O motivo mais mencionado pelos gestores, para inclusão da Saúde Bucal foi a possibilidade de mudança do modelo tecno-assistencial. Dentistas e auxiliares quando questionados sobre sua motivação, também consideraram prioritariamente a possibilidade de mudança na prática. Observou-se ainda que existem nós críticos na contratação e capacitação de recursos humanos para atuar na equipe de saúde bucal e na integração entre os profissionais de saúde. Mas o acesso está melhor, seja pela organização efetuada, seja pela expansão de equipes. Os municípios da microrregião ainda enfrentam percalços para que o funcionamento do serviço de saúde bucal, após a Saúde da Família, apresente avanços reais nas práticas de atenção. Porém, os avanços existem e estes devem ser reforçados, para consolidação do sistema de saúde e melhoria na qualidade de vida da população.

This scope of this article is to discuss the inclusion of oral health in the Brazilian Family Health Strategy in municipalities of a micro-region of southeastern Brazil, from the standpoint of human resources. Questionnaires were distributed to members of the oral health service: dentists, assistants and managers. The instrument included three dimensions: how and why they adopted the new model; how the service is being reorganized; and what advances were perceived by professionals. The most common reason mentioned by managers for the inclusion of Oral Health was the possibility of changing the technical care model. When asked about their motivation, dentists and assistants also considered the possibility of change in practices to be a priority. It was also seen that there are critical bottlenecks in the hiring and training of human resources to work in the oral health team and in integration among health professionals. Indeed, access is enhanced either by the organization or by the expansion of the teams. The municipalities of the micro-region still encounter difficulties for the functioning of the oral health service, after Family Health, to show actual progress in care practices. However, advances exist and these need to be bolstered to consolidate the health system and improve the quality of life of the population.
